Seasonal Influenza Shots Available for Notre Dame Students
Hello Parents & Guardians,
Seasonal Influenza will be available to Notre Dame Hounds students starting Monday, October 21, 2019. Please submit your form if you would like your son or daughter to receive their Season Influenzas. The Saskatchewan Health Authority requires all children aged 6 months to 17 years to have parental consent for Seasonal Influenza.
Influenza Vaccine Fact Sheet
Consent forms need to be filled out and returned to health care before your child can receive the Influenza. If you would like your child to receive the Seasonal Influenza, please fill out the form (Section 1,2 & 3) and return it back to Health Care.
